Services available Independent Living Services (ILS) and Supportive Living Services (SLS): Divine Inclusions will assist all clients with an individualized plan with services tailored to their needs, likes, and dislikes. We target skills such as but not limited to self-care & self-advocacy, community resource awareness, cooking & meal prep, cleaning, home & community safety, medical & dental service access, financial management & literacy, recreation, shopping, emergency intervention, education & vocational, employment & entrepreneurship support, and volunteer support.

Divine Inclusions will assist Individuals to increase their level of independence in the home and in the community.

Community Integration, Divine Inclusions offers real-world learning opportunities in everyday settings like libraries, banks, public offices, and job centers. We also promote personal growth through self-advocacy training, volunteer opportunities, and leisure activities that foster confidence and connection.

Our caring staff is committed to helping each individual live more independently. We provide hourly support based on each person’s needs, and ongoing encouragement to help individuals achieve successfully living on their own. Dr. Cynthia Divinity and her team, are passionate about helping individuals with disabilities and their families take control of their futures through California’s Self-Determination Program (SDP). Led by Dr. Cynthia Divinity, an experienced educator, advocate, and certified Independent Facilitator, our team provides personalized support to help you build the life you choose—based on your goals, values, and vision. As an Independent Facilitator (IF) and a General Self Determination (099) provider we specialize in:

Program Overview

The self-determination program is a voluntary, alternative way to receive services and supports. Divine Inclusions will provide support which assist Individuals with their transition from traditional services into the Self-Determination Program (SDP). Services will consist of an individualized person- centered program plan, which includes assisting the Individual to identify and meet potential SDP needs under the guidelines of the department. Services will be provided in- office, in-home, and in the community.

Person-Centered Approach  

Divine Inclusions will assist you in setting goals which aligns with the future you envision for yourself. Divine Inclusions will provide person-centered services which includes assisting, coaching, and/or training supports needed by a potential SDP participant and their family or their representative to successfully enroll into SDP. Divine Inclusion will assist individuals and their families with navigating through the SDP process increasing their ability to gain more freedom, control, and responsibility in choosing services and supports to meet their goals in life and their Individual Program Plan  IPP).

Person-Centered Plan

Divine Inclusion will support Individuals with creating a person-centered plan. The person-centered team is led by the Individual and includes the voice of their circle of support. The person-centered team is structured to ensure the Individual has a plan in place for a continual process of listening, learning, and actions that helps the Individual to secure a future of their dreams. Divine Inclusions will ensure the person-centered plan is committed to recognizing and supporting the rights of the Individual. Divine Inclusions will ensure the person-centered plan reflects the Individuals capacities, what’s important to and for, likes and dislike, and supports needed. Divine Inclusions is committed to a process-oriented approach empowering Individuals to plan their life, find their voice, work towards reaching their goals, and allowing Individuals to be the author and center of the planning process.

Self-Directed Supports  
Individualized Program Planning
  • Coordination of the person-centered plan
  • Attend and assist at the individual program planning meetings
  • Assist in identifying goals and SDP services
  • Understanding the transition from traditional services to SDP

Coordination of Services and Supports
  • Implement effective interviewing techniques.
  • Manage staff and provider onboarding/offboarding.
  • Negotiate service rates and clarify qualifications for SDP services.
  • Guide the criminal background check process for staff.
  • Promote integrity, work ethics, and time management.
  • Clarify staff classifications, benefits, and the chain of command.
  • Review resumes and manage staff expectations, schedules, and payroll.
  • Facilitate staff orientation and ongoing training.
Management of Services and Supports
  • Assist in understanding and obtaining a certified 12-month SDP budget.
  • Educate on the development and management of the SDP budget and spending plan.
  • Provide guidance on utilizing generic resources.
  • Clarify the role of the regional center as a payee of last resort
  • Educate on the legal and regulatory use of SDP funds (state and federal).
  • Facilitate communication and coordination between the regional center, providers, and staff.
Participant Requirements 
  • Divine Inclusions is available to assist Individuals of all ages and gender who has completed the self-determination orientation and is interested in transitioning from traditional services into the self-determination program.
Eligibility Requirements
  • Individuals over the age of three (3) with Developmental Disabilities who receives regional center services, who lives with family or in a community setting. Individuals who agrees to participate in an orientation for the Self-Determination Program, work with a Financial Management Services (FMS) entity to manage the individual budget, and agrees to only purchase services eligible for federal reimbursement and that are not available through a generic agency.

 

* Individuals who live in a licensed, Long-term Health Facility, such as a Skilled Nursing Facility, Developmental Center, or an Intermediate Care Facility are NOT eligible for SDP unless they are in the process of moving into the community, within the next 90 days.
